How Do Providers Decide Between IOP, PHP, and Virtual Care?

When you’re looking at mental health treatment options, terms like IOP, PHP, and virtual care can make it hard to know where you fit.

Providers don’t usually make that decision based on one symptom or how convenient a program is. They look at how much support you need, how safely you’re functioning outside of treatment, and how much your symptoms are interfering with everyday life.

Not sure which level of care makes sense for you? Contact Vitalize Behavioral Health to talk with the team about your symptoms and available treatment options.

What Is the Difference Between PHP and IOP?

Both are structured outpatient programs, but PHP generally involves more treatment time and a higher level of support.

An intensive outpatient program, or IOP, may be appropriate when someone needs more than weekly therapy but can still safely spend most of their time at home, work, or school. Vitalize’s Adult IOP generally includes several hours of treatment about three days per week.

A partial hospitalization program involves more treatment hours and may be recommended when symptoms are significantly interfering with daily functioning or someone needs closer clinical support without 24-hour hospitalization.

The Centers for Medicare & Medicaid Services describes IOP as a structured outpatient program involving at least nine hours of services per week under Medicare, while PHP is a more intensive outpatient level involving at least 20 hours per week. CMS outlines the distinction between the two levels of care here.

Where Does Virtual Care Fit In?

Virtual care describes how treatment is delivered, not necessarily how intensive it is.

For example, Vitalize’s Virtual IOP provides an intensive outpatient level of care through live video sessions. Participants still receive structured treatment throughout the week, but they can join from home instead of traveling to a treatment center.

A provider may consider virtual care when someone clinically fits IOP, can safely participate from home, has a private space and reliable technology, and can consistently attend scheduled sessions. For someone who needs more in-person monitoring or a higher level of structure, an in-person IOP or PHP may make more sense.

What Do Providers Look at During an Assessment?

There isn’t one checklist that automatically places everyone into the same program.

A clinician may look at your current symptoms, safety concerns, ability to manage daily responsibilities, previous treatment, support system, medications, and whether symptoms have recently become more or less intense.

Practical factors can matter too. Work schedules, school, caregiving, transportation, and distance may help determine whether an appropriate level of care is easier to access virtually or in person.

The recommendation can also change. Someone may start in PHP and step down to IOP as they become more stable, or move to a higher level of care if their needs increase. The goal is to match the amount and format of treatment to what will support you right now.

You don’t have to determine the right program before reaching out. Contact Vitalize Behavioral Health to complete an assessment and get guidance on the level of care that fits your current needs.

Clinical Director

I am a client-centered and ACT-focused clinician with a significant history working with children and adolescents. I deeply believe that therapy needs to be driven by the people we are working with, in order to achieve their own unique goals and personal ideals in regards to what they want their lives to be.

Areas of Specialization

  • Trauma
  • Depression
  • Anxiety
  • LGBTQ+
  • Children and Adolescents

Therapeutic Approach

I was trained as a person centered therapist through my graduate program and quickly gravitated towards acceptance and commitment therapy, which is a third wave behavioral therapy that focuses on acceptance of emotions and thoughts, coping effectively and committing to living our lives by our own values and ideals. I also utilize play therapy with younger clients that allows them to explore and process their emotions and struggles through a developmentally appropriate process. What clients can expect more than anything else is receiving therapy that is tailored to their needs. This would include exploring what gets in the way of them living how they want in a deep, meaningful way, developing skills and strategies that are tailored to their experience, and moving in a significant way towards the behaviors and goals they desire for themselves.

Education and Credentials

  • B.A. Psychology
  • M.S. Counseling

About Me

Primarily I gravitated towards becoming a therapist as I grew up with family members who needed mental health care in significant ways and I saw first hand how good, quality care benefitted them. I also attended therapy as a child and teenager and will be forever grateful that I saw competent, ethical therapists. Beyond that I am also a lifelong athlete who has seen the benefits of sports and meaningful experiences on people’s lives. I have been a whitewater river raft guide for the past 15 years and have worked on the Kings River and the American River.

I became a therapist after starting my work as an emergency response advocate, where I supported individuals and families during some of their most difficult moments. Over time, I realized my true calling was in helping people not just survive those experiences, but heal from them, and that’s what led me to the deeper, ongoing work of therapy.
