Continuum of Care
Mental health challenges are as diverse as the people who face them. That’s why we’re committed to providing a well-rounded continuum of care with a broad range of services available under one practice, from the comforting support of one-on-one therapy to the immersive experience of an intensive outpatient program.
We aim to ensure that every patient has continual access to the appropriate care, regardless of how their needs or circumstances may change. Thanks to our data-driven approach and robust care options, we are positioned to adapt and cater to an individual’s evolving needs. That way, during life’s most challenging moments, patients don’t need to seek a new provider; the Vitalize team remains a reliable source to guide them.
Comprehensive Programs
Outpatient Psychiatry and Medication Management
Our team of psychiatric service providers are here to support you through a comprehensive evaluation process to identify your unique psychiatric needs. This assessment will help us create a tailored medication management plan to guide you on your path to mental wellness.
Intensive Outpatient Program (IOP)
This highly structured program is for individuals who require more support than traditional outpatient therapy. It involves multiple weekly sessions that allow patients to maintain their daily routines.
Partial Hospitalization Program (PHP)
In-depth treatment typically involves daily sessions, usually spanning several hours daily. Patients are able to return home each night, making it a thorough and flexible choice for those in need of more intensive care than an IOP.
